Molar Crown on 4.0mm Dental Implant

I have inherited a patient who has a fully integrated 4.0mmx13mm in the edentulous space of #14.  The mesiodistal edentulous space is 12mm wide, the buccal and lingual bone support around the implant is over 2mm and the implant is located right in the middle of the edentulous space.  The patient was told previously that he would be receiving a crown that would fill the dentulous space.  The only acceptable treatment option that I can think of is a very narrow crown (buccolingually) spanning the edentulous space.  The patient does not want any more surgery.  I am open to suggestions.

Dental Implant Barrier Membranes

Barrier Membranes certainly have their place in dental implant placement.  I much prefer a resorbable membrane because of no second surgery, but if not primary closure the membrane picks up the stuff in the patients mouth and smells.  I am having very nice success using Gel Foam over grafting material with primary closure.  Has anyone else done this method.  What membranes have been successful without primary closure?

Dental Implant Failure Treatment

I have a patient whose dental implants are completely stable but there is Peri-Implantitis.  There has to be an eay way to treat this and prevent this type of situation in the future with other patients.  The occlusion is fine and their home care is ok. Any thoughts?

Dental Implant Temporization Problems

A patient who required multiple dental implants is four months into the maxillary case and did not want to wear anything removable.  I thought I was such a smart guy by staging the case using some remaining  natural teeth to support the fixed temporary.  Well the patient came in with his heat processed temporaries broken off and in pieces.  The teeth are down to the gum line. I thought about IMTEC transitionals but I am not sure they will stand up to the abuse.  Only two months left and I am thinking  of pushing the time but it was type two and three bone.  Any thoughts?  I cemented the temps on with Panavia until Thursday.

Dental Implant Abutment Retention

So many of us are getting away from screws coming through the occlusal surface for our dental implant supported cronws because of esthetics.  But even torqued down screws have became loose under cemented implant crowns.  I have checked the occlusion and even placed cement in the screw holes.
Any thoughts?

Dental Implant Failure

Placed a dental implant and it was type one bone, no pain for two weeks and then intense pain.  The dental implant was surrounded by granulation tissue and no pus.  The site was difficult to get numb and very inflamed.  Thought it was an isolated case and then five more.  Has anyone seen this?
